Could not create default usage

What causes this issue?

When creating a new value assignment, the system automatically tries to
create a standard usage. The usage could not be created or not be
created completely.

System Response

The system issues an error message and will not allow you to continue with this transaction until the error is resolved.

How to fix this error?

Check the usage assigned to the current value assignment and correct
it, if necessary.
