Operation number &/&/&/& is not unique

What causes this issue?

The operation &V4& in sequence &V3& of group &V1&, group counter &V2&
is not unique. Processing is only possible if all operations in a
sequence have unique numbers.

How to fix this error?

Please correct the operation numbers.
