Region &1 does not match district name &2 in country &3

What causes this issue?

There are several cities for the city name &V2& in country &V3%.
However the region &V2& is not assigned to any of these cities.

System Response

The system issues an error message and will not allow you to continue with this transaction until the error is resolved.
