Quality notification was activated automatically using defect class

What causes this issue?

The recorded defect(s) are assigned to a defect class that causes a
quality notification to be automatically set in process.
All defects in an inspection lot, operation, or characteristics in an
operation are documented in a quality notification. The quality
notification is initially "outstanding" that is, it is simply used
to document the defects that have been found. After it has been set in
process, the defects must then be processed as a quality notification.

The system issues an error message and will not allow you to continue with this transaction until the error is resolved.
